If a friend of family member is taking your pet on your behalf, they MUST be the person to attend the appointment to have the certificate issued. You will also need to provide a signed statement that allows them to travel with your pet which includes their details, your details and your pet's microchip number. Animal health certificates can be issued a maximum of 10 days before arrival in the EU. Please note there is a £48.50 surcharge when less than 5 working days notice is given for an animal health certificate to be prepared before issue.

The proof of rabies vaccination MUST include microchip number, date of vaccination, vaccine manufacturer name, vaccine batch number and signature of veterinary surgeon.

Payment*
- I understand I will be contacted for payment and my AHC will not be processed until payment has been taken. This must be done at least 5 days before the certificate needs to be issued, otherwise I understand there is a £48.50 surcharge for urgent certificates. I understand that payment is non-refundable.
Appointment*
- I understand I will need to book an appointment with the practice a maximum of 10 days before arrival in my destination country in order for my pet's microchip to be scanned and for the certificate to be completed. Please note the person travelling with the pet must attend this appointment in order to sign the owner declaration.
